After a bit of online digging and a call to SoliCo (the name I found stamped on the top), I have an answer.
It is SoliCo P/N 3450-4-50-19314. Unfortunately, they no longer stock it and only do runs of 250.
HOWEVER, Custom Marine Services (cmsquick.com) has them in stock and ready to ship. They're $39 each. A call to them and he knew exactly what I was talking about and got them out same day.
